Too much product, or product placed too far from the lip border, affects speech, drinking from a cup and the way you smile. This is a treatment where technique matters more than product.

We look at how much upper lip you show at rest and when you smile, whether your concern is actually lip volume rather than lip position, and whether a flip alone will get you what you want. Sometimes the honest answer is that it will not, and we will say so.

Lip flip or lip filler — which do you actually want?
These two treatments get confused constantly, and they do different things. Choosing the wrong one is the most common reason people are disappointed with the result.
- Lip flip — relaxes the muscle around the upper lip so that it rolls slightly outward, showing more of the pink of the lip. It adds no volume whatsoever. It suits people whose upper lip disappears when they smile, or who show a lot of gum. Effect appears over several days and lasts around two to three months.
- Lip filler — adds actual volume with hyaluronic acid. Visible immediately, lasts far longer, and is what you want if your lips are genuinely thin rather than simply rolling inward. See also thin lips.
- Both together — a common combination, giving volume from filler and a better-defined upper lip line from the flip.

Lip flip: common questions
How long does a lip flip last?
Usually around two to three months — shorter than most other botulinum toxin treatments, because only a few units are used in a small, mobile muscle. Most patients who like the effect repeat it three or four times a year.
How long does it take to work?
It is not immediate. You will normally start to see the change at around three to five days, with the full effect by two weeks. If you are having it for an event, book at least two weeks ahead, not the week of.
Does a lip flip make your lips bigger?
No, and this is the single biggest misunderstanding about the treatment. It adds no volume at all. It changes the position of the upper lip so more of the pink shows, which reads as a fuller lip in photographs but does not increase size. If you want genuinely bigger lips you want filler.
Will it affect my speech or drinking?
It can if it is overdone. A small number of people notice difficulty with sounds like “p” and “b”, or with drinking through a straw or from a bottle, in the first couple of weeks. Using a conservative dose with accurate placement makes this uncommon, and it always wears off. Tell us if you are a singer, a wind instrument player or a professional speaker — we may advise against it entirely.
Does it help a gummy smile?
It can help a mild gummy smile, since relaxing the muscle stops the upper lip lifting so high. More pronounced cases are treated with injections into a different muscle group, and some are down to tooth and jaw proportions rather than muscle, in which case an injectable is not the answer. Is it safe?
Botulinum toxin around the mouth is a well-established treatment when performed by a trained medical injector who assesses you first. The risks are dose and placement related — temporary asymmetry, difficulty with certain sounds, or an uneven smile — and all are temporary because the product wears off. It is not suitable in pregnancy or breastfeeding, or with certain neuromuscular conditions, which we check at consultation.
